Host/Recruiter Request Form: I am interested becoming a Host/Recruiter for a workshop

I understand that each workshop is 2 hours long. I understand that there are 3 kinds of Hoop Dance workshops offered that I may choose from. I understand that workshops are offered for beginner & intermediate Hoopers. I understand that I can host as many Hoop Dance workshops as I wish, within 1 week. I understand that I am responsible for finding/providing a facility, recruiting students & taking care of pre-registration money. I understand that Betty Hoops must be PAID IN FULL for all students 1 month prior to the workshop/s. I understand that if my facility holds 20 students (without hoops) comfortably, then it will hold no more than 12 students (with hoops) comfortably. I understand that there must be a minimum of 12 students enrolled for each workshop. Or if my space is too small to fit 12 Hoopers, I must host more than 1 workshop that same day. I understand that as a Host/Recruiter, I will receive 1 Complimentary Admission, bumper sticker, 1Betty Hoop® of choice & 50% off a T shirt. I understand that submission of this request does not guarantee that a Hoop Dance Workshop will be held at my facility. I understand that if I fail to honor the above items, the workshop may be postponed or cancelled. I am ecstatic about working closely with Betty Hoops to make sure that our Hoop Dance workshops are awesomely memorable! As a Host/Recruiter, I have read, understand and accept the above terms and responsibilities of this request. *Betty Hoops® will help to market your event. You will receive flyers, registration forms & interested attendee contacts in your area.
